Brow Lamination & Tint Consent Form

I consent to having an eyebrow lamination (perm) and tint and/or retouching of an eyebrow and tint applied to the natural brow performed by the certified lash artist listed below.
I understand, in rare occasions, the risks associated with the service.
I understand that irritation, redness, itching, burning, and allergic reactions may occur during and/or immediately after the service.
I understand and agree that if any of these issues persist, I will contact the certified lash artist and/or consult with a physician at my own expense.
I agree to the aftercare instructions explained to me and understand that failure to follow these instructions may result in the decreased life of the brow lamination and tint.
I was given the opportunity to ask the lash certified artist any questions I have regarding the service, and I have had those questions answered to my satisfaction.
I understand that there are no refunds given for eyebrow services for any reason.
I consent to having my before and after photos taken for advertising purposes, and that these photos may be posted on social media sites.
