Lash Extension Consent form

I consent to the application, removal, and/or re-touching of the eyelash extensions to the natural lash.
I understand, in rare occasions, the risks associated with the application and/or removal of eyelash extensions to the natural lash.
I understand that irritation, redness, itching, and watering of the eyes may occur during and/or immediately after the service.
I understand that if any of these issues persist, I will contact the certified lash artist within 24 hours and may have the eyelash extensions removed at no additional cost.
I understand that waiting longer than 4 weeks to receive a lash fill will result in needing a full set, at full price during the next service.
I understand that there are no refunds given for lash services for any reason.
I understand and consent to laying in a reclined position while having my eyes closed and covered for the duration of approximately 60–120 minutes during the procedure. Times may vary depending on the type and number of eyelashes applied.
I understand the aftercare instructions given and realize that failure to follow these instructions may result in the decreased life of lash extensions and/or damage the natural lash.
I consent to having my before and after photos taken for advertising purposes, and that these photos may be posted on social media sites.
I understand that I will be charged a 50% late cancelation fee for any cancellations within 24 hours of the scheduled appointment. No-shows will be charged 100% of service fee.
